SUMMARY: Depot medroxyprogesterone acetate (DMPA) is a highly effective injectable contraceptive that provides anonymity and has a four-times-per-year dose schedule, making it appealing to a wide range of users, especially adolescents. Despite the fact that DMPA use is linked to a loss of bone mineral density (BMD), existing longitudinal and cross-sectional evidence indicates that BMD recovers after DMPA is stopped. There are no high-quality data on whether DMPA affects fracture risk in adolescents or adults later in life, which is a significant clinical concern. The effect of DMPA on BMD and the potential for fractures does not deter doctors from prescribing it or continuing to use it for more than two years. The health risks associated with DMPA’s bone effects must be weighed against a woman’s probability of becoming pregnant using other methods or none at all, as well as the known negative health and social implications of unintended pregnancy, especially among adolescents. Health care professionals should address the advantages and drawbacks of DMPA with women and adolescents who are considering starting or continuing to use the procedure, as well as the FDA’s “black box” alert and use clinical judgment to determine whether or not the method is suitable.
