The liver is one of the body’s largest and most versatile organs. It converts sugars, proteins, and fats in our diet into body-friendly substances and delivers them to our cells. The liver is an immunological organ that is important for blood detoxification in addition to its function in human metabolism. The liver, in particular, is the only internal organ that can regenerate to its original size with only 25% of its original mass.
Liver diseases are one of the world’s most serious health issues and a leading cause of death. Fatty liver disease, liver cancer, or hepatitis affect at least five million people in Germany alone. Despite the liver’s critical role in human health, the variety of individual liver cell types and the molecular and cellular processes that govern them in both healthy and diseased tissue has yet to be thoroughly explored.

Understanding the microenvironmental conditions that control the phenotype of human hepatocytes in vitro will require the generation of differentiated normal adult human hepatocytes, which is a major challenge. We describe optimized 3D culture conditions for primary human hepatocytes (PHH) to cause two waves of proliferation in this paper, and we identify matrix stiffness and cell–cell interactions as the key actors required for this proliferation. The matrix stiffness modulates DNA replication and overexpression of cell cycle markers, while PHH cultured in 3D without prior cellular interactions did not proliferate. Furthermore, we discovered that after transient inhibition of the MAPK MER1/2-ERK1/2 signaling pathway, PHH undergo an additional cell cycle. Hepatocytes cultured in collagen form characteristic hollow spheroids that can sustain survival, cell polarity, and hepatic differentiation for at least 28 days in culture. Using transcriptomic analysis and functional experiments, we were able to show that proliferating cells are mature hepatocytes with high detoxication capacities. Finally, the Hepoid advanced 3D model mentioned here is particularly useful for obtaining normal human proliferating hepatocytes. It’s a promising method for many pharmacological and biotechnological applications because it allows for simultaneous proliferation and differentiation.

Since chronic hepatitis A and C infections, as well as alcohol abuse, are the leading causes of liver cancer, it is a highly preventable disease.
